Well it would seem that quality control is practically non existent within Big Pharma, as it is in many other industries. Just how did thousands,if not millions of crap flu shots get out the door?  Profits, I suppose.

I remember,many decades ago, working in the Chrysler factory in Linwood. I was given the job to find a piece which fitted in the machine to made reverse gears. I could not get one anywhere.

So they just sent the cars out to the showrooms anyway. Mugs were buying new cars with no reverse. When the folks complained to the showroom they were shocked. They knew nothing about it either. Eventually I had a bit specially made in America and flown over. They then sent out the reverse gear hones for the showrooms to fit them into the gearboxes. Why? profits. Units were out the door and income was coming in.
