Supposedly secret rooms were placed with main offices of all the telecom firms in America years ago !! These are for listening and/or recording numbers of interest.
In Britain, the main company British Telecom have a different system. A low level spook would phone a designated BT engineer, who would then go into an exchange and tap any lines required. This is all done with a nod and a wink. No Court Orders are ever applied for by the spooks.
